In order for your son to join our pack he must complete an application that will be given to you.  Below are the additional forms required.

Annual Medical Record Form

All scouts must complete Part A & B of the BSA Health History Form by the first meeting he attends.  For your convenience this form is available below for you to download and print.

Permission Slip Form

All scouts must complete a permission slip before participating in any event other than a Pack or Den Meetings. For your convenience this form is available below for you to download and print.

Cub Scout Web Consent Form

Clothing

Shirt -     Official Cub Scout Uniform Shirt at least one size larger.  We recommend short sleeve and your son wear 
              something underneath it during colder weather.
Pants -   Any navy blue pants that match the shirt
Hat -      Rank appropriate
Belt -     Official Cub Scout Belt (needed for belt loop achievements)
